The Living Waters Canada Ambassadors’ Academy requires that every church send two participants to be trained together — a primary applicant and an apprentice applicant.

Both the primary applicant and apprentice applicant are required to complete this application form, and both are also required to enter the full name of their co-applicant below.

Step 5 of 9 — Doctrinal Affirmations
Please indicate whether you agree with the following statements.
Jesus Christ—fully God and fully Man—lived a perfect, sinless life. *
I was born with a sin nature. *
I have sinned against God by violating His Law. *
Apart from salvation through Jesus Christ, I would rightly spend eternity in Hell as the just punishment for my sins against God. *
I have repented and have placed my faith and trust in Jesus Christ for my salvation. *
I am saved by the grace of God alone, through faith alone, in Jesus Christ alone. I am saved apart from any and all good works. *
Jesus is the way, and the truth, and the life; no one comes to the Father but through Him. There is no other name under Heaven by which anyone can be saved. *
I am pursuing holiness and godliness in my life, as an outward manifestation of the fruit of the Spirit in my life. I do this, not as a means of earning salvation, but rather as a result of the free gift of eternal life I have received through Jesus Christ my Lord. *
Step 6 of 9 — Statement of Faith

We believe the Bible to be the inspired, infallible, authoritative, and inerrant Word of God (2 Timothy 3:15–17; 2 Peter 1:21). *
We believe there is one God, eternally existing in three persons, each fully God: the Father, Son, and Holy Spirit (Genesis 1:1; Deuteronomy 6:4; Matthew 28:19; John 10:30). *
We believe in the deity of Jesus Christ (John 10:33); His virgin birth (Isaiah 7:14; Matthew 1:23; Luke 1:34,35); His sinless life (Hebrews 4:15; 7:26); His miracles (John 2:11); His vicarious and atoning death (1 Corinthians 15:3; Ephesians 1:7; Hebrews 2:9); His bodily resurrection (John 11:25; 1 Corinthians 15:4); His ascension to the right hand of the Father (Mark 16:19); and His personal future return to earth in power and glory (Acts 1:11; Revelation 19:11–16). *
We believe in the absolute necessity of regeneration by the Holy Spirit for salvation because of the exceeding sinfulness of the human nature, and that all are justified on the single ground of faith in the shed blood of Christ, and that only by God’s grace through faith alone are we saved (John 3:16–19; 5:24, Romans 3:23; 5:8,9; Ephesians 2:8–10; Titus 3:5). *
We believe in the resurrection of both the saved and the lost; those who are saved unto the resurrection of life, and those who are not unto the resurrection of damnation (John 5:28,29; Acts 24:15). *
We believe in the spiritual unity of all believers in our Lord Jesus Christ (Romans 8:9; 1 Corinthians 12:12,13; Galatians 3:26–28). *
We believe in the present ministry of the Holy Spirit by whose indwelling the Christian is enabled to live a godly life (Romans 8:13,14; 1 Corinthians 3:16; 6:19,20; Ephesians 4:30; 5:18). *
We believe that as Christians we are examples of the love of God in this world. It is this (agape) love that we desire to possess and without which we have no right to call ourselves Christians (1 John 4:16,17). *
We believe it is the mandate of the whole Church to go into the whole world and preach the gospel to every living creature (Matthew 28:19,20; Mark 16:15). *
Step 7 of 9 — Doctrinal Denials
Please indicate your view on the following doctrinal statements. A “No” answer means you are in agreement with the doctrine. All questions must be answered or your application will not be processed.
I deny the belief system known as Moral Government Theology. (A belief that maintains that God is not immutable but changes His mind, that He does not exercise sovereign control over earthly matters, that He does not know all future events—particularly the free-will choices of individuals, etc. It denies that the atonement pays for our sins, denies Jesus’ substitutionary death, and denies the imputed righteousness of Christ to the believer. It asserts that people are capable of keeping the whole Law of God and that there is no depravity of human nature.) *
I deny the doctrine known as Sinless Perfectionism (the belief that Christians can attain a state of completed sanctification or moral perfection in this earthly life). *
I deny the doctrine known as Open Theism (the belief, in part, that God does not have complete foreknowledge of the future). *
I deny the belief in Purgatory. *
I deny the doctrine known as Baptismal Regeneration (baptism as a requirement for salvation). *
I deny the requirement of speaking in tongues as evidence of authentic salvation (this does not pertain to whether this particular gift of the Spirit is available to Christians today). *
I deny the theory of Theistic Evolution (the belief that God initiated life on earth and allowed evolutionary principles to form mankind). *
I deny the doctrine known as Annihilationism (non-existence after death for sinners who reject the gift of eternal life through Christ; Annihilationism is a denial of the doctrine of Hell). *
Step 8 of 9 — Statement of Unity
The Ambassadors’ Academy brings together Christians from many different church traditions and denominations. Please read and respond to the following statements about unity in the gospel.
I believe that all true Christians are united in one body — the Church — regardless of denominational differences, and that this unity is centred on the person and work of Jesus Christ. *
I am willing to work alongside and learn from Christians from other denominations and traditions during the Academy, for the sake of the gospel. *
I commit to maintaining a spirit of humility, love, and grace toward fellow participants, staff, and all those I will witness to during the Academy. *
I understand that the Academy is not a platform for promoting specific denominational distinctives, and I commit to keeping the focus on the biblical gospel. *

The final step of your application requires a pastoral reference. This reference form is to be completed by your pastoral leader contact with your church who knows you and your walk with the Lord well.
